Why can the tool not find the code in my photo?

Blur, glare, a steep angle or a cropped edge all prevent detection. Retake the picture square-on with the whole code and its margin visible.

Is it safe to scan a QR code from someone I do not know?

Can it read a damaged or partly covered code?

Sometimes. QR codes carry error correction, so small damage is recoverable, but a missing corner marker usually is not.
